Data Section
The top of the data section (area B) contains information to identify which student is currently being administered.

Area D is where the office visit information is maintained. This view is the main view of the application. It will be used the majority of the day. This view provides the means of adding and updating office visit information.

Schedule Follow-up Visit ( Area F)
This hyperlink is a special feature to provide an easy means of scheduling follow-up visits. The advantage of using this feature (vs. just adding a new office visit) is that this feature will automatically copy information from this visit to the follow-up visit. Also the two office visits will be linked together. To take full advantage of this feature it is recommended that the information in this visit be completely entered (especially the treatment) before the follow-up visit is scheduled.
Additional Links
